HOWL contains explicit, consensual sexual content and nudity, including masturbation, oral sex, manual genital stimulation, digital vaginal penetration, orgasm, orgasm denial and edging, consensual sexual use of a smooth steel instrument by the heroine on her own body while a partner watches, a male knot displayed without contact, and external ejaculation. It also contains frank anatomical language and brief, direct sexual boundary language.

The story includes omegaverse presentation, scent, biological quieting, involuntary physical symptoms, and discussion of bonds and institutional control over omegas. No sexual contact is used as medical treatment.

Other content includes medical examinations; needles and blood draws; fever, collapse, sensory distress, blood, illness, and injury care; a consensual medic-apprentice relationship with a professional power imbalance; family coercion, caretaker exploitation, sleep deprivation, parentification, and emotional neglect; isolation during severe winter weather; institutional control and threatened reassignment; grief, bereavement, and moral injury.
